Summary/Abstract: Cardiovascular diseases are a major cause of morbidity and mortality worldwide, necessitating the search for new ways to address and expand prevention activities to treat these diseases. Heart failure (CH) is a chronic, progressive disease that increases in frequency with age, is associated with comorbidities and is a cause of frequent rehospitalizations. This results in a worse prognosis, disability, economic burden for the society, and a long-term need for health care. There are various options for dealing with the severe and progressive nature of heart failure. Each of the contributing factors to the disease can be controlled and optimized to reduce unwanted hospitalizations, increase patient longevity and quality of life. Objective: is the establishment of an outpatient clinic for HF, where adequate control and timely intervention on the factors favoring the exacerbation of HF will be checked. A significant part of the activities of the nurse in such office is focused on optimal control of body weight, blood pressure, diet and exercise regime, and unhealthy life-style habits. The HF management emphasizes the significant role of the nurse capable of controlling the process of self-management, decision making and creating organizational support skills. The Cardiac Nursing Center can improve the long-term status of patients and be an effective unit in promotion and prevention activities.
