Summary/Abstract: Inclusion is a way of thinking and acting that allows each individual to feel accepted and safe. Inclusive education refers to adequate education for all children at each education level. Children’s transition into school is an emotional and complex period for any family. This transition can be even more challenging for families where are children who have additional support needs. Various studies have shown that there is a vertical discontinuity between kindergartens and schools, and that children may experience anxiety and stress if the transition has not been adequately implemented. For this reason, in many countries there is tendency of forming a transition team, which is the core group that facilitates and makes plan of transition process. These collaborative teams often include educators, teachers, principals, parents and other community representatives. Bearing in mind the national and international perspective, this paper will present various examples of transition plans, protocols, guidelines and documentation list, which clearly separate the roles of each member of the team, his contribution and responsibilities to the transition process into school. Gain of transition planning in inclusion practices are: definition of the position and role of each transition participant, starting from the child, his family, representatives of the kindergarten and school, but also the wider community: identification of the difficulties in moving from one to another education level; analysis of appropriate measures to provide maximum support for the child, his family and the school preparation for the child acceptance.
