Summary/Abstract: The presented contribution brings translation of an official Order of Lombardy Imperial and Royal Government from 13th August 1821, about overall inoculation of population against smallpox. The text of contribution is divided into a short historical outline of the smallpox pandemic and methods of vaccination against it, up to its vanishing in the last century. Then there follows the translation of the original Order from 1821, divided into three parts: the first one contains the order of vaccination from the choice of certified medical doctors to the description of their duties, reports to superior authorities on the level of villages, towns, provinces and the supreme governmental medical supervisory body. The second part is devoted to the instructions in 32 articles for authorized medical doctors, containing instructions from the diagnosis of smallpox, course of the illness, transfer of suppurating substance from smallpox which was used as an inoculant, as well as a description of the method how to apply it to a vaccinated individual. The final part of the text is completed with supplements (tables and surveys with individual columns translated), serving to outline the data concerning vaccination, about the number of inoculated individuals, success of the vaccination and a survey of executing the vaccination in individual villages, towns, districts up to provinces, and the data gathered by vaccinating medical doctors during inoculation.
