Summary/Abstract: Deep caries is a disease of the hard dental tissues that is characterized by great tissue loss and destruction of the crown, with the possibility of exposing the pulp. It occurs as a result of the progression of the demineralization process. The etiology of deep caries is associated with unhealed secondary caries, unhealed acute lesions, secondary caries, recurrent caries under restorations, residual caries. Caries is one of the most common diseases of the hard tooth tissues today, whose prevalence despite modern standards of therapy and preventive measures is still growing. Caries by definition is a multifactorial, multicausal disease that affects the hard tooth tissues, which leads to destruction and demineralization of the organic and inorganic part of the tooth and the formation of a cavity. As demineralization progresses from the enamel into the dentin, very deep cavities form up to the pulp, which if not repaired in time lead to the formation of deep caries and endangerment of the pulp. Caries profunda is characterized by a large loss of hard tooth tissue and can lead to the opening of the pulp chamber. Depending on whether the removal of cariously altered dentin leaves a thin but still healthy layer of dentin between the pulp and the cavity or the opening of the pulp occurs, we distinguish between simple and complicated caries profunda. Today there are many different materials, which according to the indications allow us different treatment and access to deep carious lesions. Indirect coverage and direct coverage of the pulp are used as the most commonly used methods in everyday practice. Therapeutic procedures for caries profunda are primarily correlated with the depth of the caries and the preservation of the pulp vitality. Today, various methods, techniques and means are used for that purpose, which preserves the vitality of the pulp. caries coverage; b) Partial-selective caries removal (softened caries on the pulp is not removed) includes the following techniques: Caries removal step by step by chemical means, Indirect coating-but with direct restoration, ie in one session; c ) Complete removal of the soft tissue with the fact that if the pulp is exposed we have three techniques of choice: Direct pulp coverage, Pulpotomy and Pulpectomy.
