Summary/Abstract: Introduction: Implementation of evaluation tools in clinical practice is necessary for identify the symptoms of functional decline in elderly. Using a comprehensive geriatric assessment we find a decrease functional status, patient frailty and other disorders. Methodics: We used the methodology of analysis of the documents that were available in the databases Medline, ProQuest, EBSCO, Scopus. We were interesting in their validity, reliability and standardization. Results and Discusion: Age, lowered functional status, cognitive impairment and disability in instrumental activities of daily living (IAD L) are criteria for selection of screening instruments for elderly patients. Studies have shown that 30 to 60 % of elderly people develop new depending on the activity of daily living (AD L) during their stay in hospital. Adverse consequences for the health of mortality, longer hospital stays, placement in a nursing home and increase depending on the elderly. Of scales take into account not only the physical condition of the elderly, but also the overall quality of life. Conclusion: Evaluation is the first step to identify patients at risk of functional decline and the overall treatment is dependent on the potential of. Health professionals have an important role in the implementation of evaluation tools in clinical practice.
