Summary/Abstract: In 2014-2015 the Institute for Dignified Ageing Diakonie of the Evangelical Church of Czech Brethren conducted a project aimed at supporting the development of community health and social services with a focus on regional and, especially, municipal government authorities. The final goal of the project was to support the development of health and social services provided within the community and home environments to persons with reduced selfsufficiency and their families and to improve the level of continuity of health and social services at the local level.
