Summary/Abstract: We focus on language practices in health and medico - social so-called "total institutions" or "closed environments" (nursing homes and institutions for mental retardation), that in the context of culture and language mediation activities Picardy and computer and / or the Internet. First, we open the debate on how to do research on these lands that engage the researcher in an ethical process . On the other hand , we understand these institutions as places in which communication situations reveal relationships with variable geometry and more generally of tensions related to the construction of the medico- social work field. Finally, we question and (re) consider the role of mediation and associated language practices in these areas. Indeed, some institutions develop mediation activities that promote a "total communication", ie multi- forms: oral, verbal, para- verbal where physical and symbolic objects are resources from the environment at heart interaction. Therefore, institutions will open to the outside (the ordinary world versus the closed environment of institutions).
