Summary/Abstract: The public associates the health care reform in Bulgaria with the social health insurance model aimed at improving personal responsibility for health, solidarity and cost effectiveness. The instant contribution deals with the long-term financial sustainability of health insurance since its introduction in Bulgaria. Three controversial dimensions of financial sustainability have been discussed. Firstly, the paper touches on the importance of the ―pure‖ market in Bulgaria, the lowest average income country in the EU. Secondly, it explores how certain government failures negatively affect the budget of the National Health Insurance Fund. Thirdly, this research analyses the asymmetrical structure of health insurance payments to health care providers, which does not contribute to health promotion and disease prevention in Bulgaria.
