Summary/Abstract: The monograph touches on issues related to management in health care in the broadest sense. This is another scientific publication that is the aftermath of a series of conferences under the title OCHRONA ZDROWIA I GOSPODARKA. This book addresses the consequences of socially undesirable behaviors and phenomena for the health care system and the economy, the position of the patient in the health care system, the problems of management of health care facilities or external determinants of the impact on public health and the health care system. The monograph is dedicated to the memory of Prof. Kazimierz Ryć, an outstanding economist, scientist, one of the initiators of the establishment of the Center for Health Care Management at the Faculty of Management of the University of Warsaw, and the scientific editor of all the monographs in the series published so far.

- Summary/Abstract:Smoking and the use of nicotine products both in Poland and in the world remain at a high level. Current research shows the emergence of new smoking trends and the emergence of innovative tobacco and nicotine products that appear to be safe and harmless. However, scientists point out that the nicotine contained in them can lead to severe addiction, and the harmful chemicals they include lead to negative health effects for their consumers in the long run. This study presents the properties of traditional and innovative tobacco products, the health consequences of smoking, and selected economic aspects related to both the functioning of the tobacco industry, revenues to the state budget due to excise duty on tobacco products, and the health effects of smoking in the context of public health. Both the European Union and its member states (including Poland) take legislative measures to reduce the health and social risk posed by tobacco products and minimize the harm caused by their use. A significant problem is a lack or low degree of regulation of the circulation and use of new types of tobacco and nicotine products, which, according to the latest research, are also harmful, especially for young people who have easy access to them.

- Summary/Abstract:In most countries of the world, an increase in the consumption of alcoholic beverages is observed. An expression of this trend in Poland is both the systematically growing level of household spending on the purchase of alcoholic products, as well as the growing revenues to the state budget from income tax from alcohol. Excessive consumption of alcohol by society has a number of negative consequences in the economic, social and medical spheres. It is a risk factor for many diseases, generates criminal activity, has a significant share among the causes of road accidents, and in the case of the drinking population leads to alcohol dependence.

- Summary/Abstract:There has been a rapid increase in the prevalence of overweight and obesity among people worldwide in recent times. Excessive body weight causes many chronic diseases and hinders proper functioning in society. Obesity treatment generates costs that can be divided into direct (related to expensive pharmacotherapy, hospitalization or outpatient treatment) and indirect, resulting from a decrease in productivity and numerous absences from work. The increase in the number of obese people is correlated with the increase in the costs of prevention and treatment of excess body weight and its consequences, which burden budgets of countries around the world every year.

- Summary/Abstract:Patients and patient organizations are becoming increasingly involved in health policy and operations of health care. The value of such activity plays a particularly important role in the area of chronic diseases such as cancer or rare diseases, where patients suffer from limited access to effective therapies due to lengthy clinical trials, complex regulatory procedures of drug registration and reimbursement. It has been suggested that input of patient organizations has a significant impact on reducing uncertainty and decision risk, throughout the entire process, beginning from the clinical trials, through engagement as experts in their diseases in registration and reimbursement, up to routine clinical practice applications. This comprehensive review examines the areas of patient organizations involvement at crucial stages of public healthcare operations. At present, activities of patient organizations seem to be closely related to the lifecycle of a medical product on the health care market and coverings following areas: psychological and emotional support, financial and non-financial assistance to patients, knowledge gathering and sharing, raising public awareness of their disease, participation in the design and conduct of clinical trials, early access programs, engaging in reimbursement decision making and advocacy aimed at regulatory changes. All inputs of patient organizations listed above generate significant impact on the health care system and shaping of drug policies. The findings demonstrate that involvement of patients, their families and patient organizations exist at many levels of the health care system. Due to study limitations, the extent and effectiveness of patient’s organization various initiatives have yet to be explicitly explored. Moreover, the existing input requires further studies to evaluate the level of engagement.

- Summary/Abstract:The purpose of this chapter was to critically analyze the codes of ethics of currently operating health care medical institutions in the state sector in Poland, as well as to identify appropriate solutions for their standardization in order to create a unified code of ethics that would combine the code of business ethics, the code of conduct for employees, the code of professional practice, the set of rights and duties into a whole, thus creating a basic mechanism to ensure full professionalism in their operation. This work analyzes, among other things, the existing rights and obligations of the patient, interpersonal staff/patient relations, standards of ethical behavior for management, and the issue of social responsibility. Unfortunately, the analysis is not detailed due to the small number of factual analyses and studies available, as well as the multifaceted nature of the subject matter, which requires effective communication-participation of all professional communities, a baseline analysis, knowledge of laws and regulations, estimation of risks, or knowledge of the industry and its market situation.

- Summary/Abstract:This chapter proposes the use of narrative medicine for the purposes of health care management. Narrative medicine enables the supplementation of clinical evidence based medicine with additional reflection and empathy, which contributes to achieving higher levels of professionalism. This part of the book presents the narrative perspective, established in four specific examples of stories on the Caesarean section. It addresses issues of teamwork, the role of information flow between team members, the hierarchy of positions, the assignment of tasks based on knowledge and skills, mutual control and an in-depth assessment of the current obstetric situation, as well as the affirmation of the continuity of care. The analysis based on examples allows us to accurately trace the events from the perspective of individual characters, along with the genesis of possible shortcomings and the formulation of universal conclusions. As a result, efforts can be made to achieve the best use of the potential of the whole medical team, the optimal division of tasks according to competencies and the increase of the quality and efficiency of the provided work. High interpersonal skills assist in appropriate co-operation between team members representing various medical professions.

- Summary/Abstract:Socio-economic analysis (SEA) of health safety is the approach to describing and analysing all relevant impacts – positive and negative – of restricting or non-restricting and granting or refusing authorisation for chemicals in the European Union (EU) market. SEA provides the evidence and documents on whether the socio-economic benefits of continued use or restricted use of chemicals outweigh or not the risk of chemicals for human health and the environment. The aim of SEA as a method of weighing up the arguments of action for society is to play the crucial role in restrictions and authorisations of the chemicals under the Registration, Evaluation, Authorisation and Restriction of Chemicals Act (REACH) in the EU. Regarding the restrictions proposals, such regulations contain a description of the risks, health and environmental benefits, associated costs and other socio-economic impacts. An application for chemicals authorisation should answer the question if the socio-economic benefits outweigh the risks to human health and the environment arising from the use of the substance. The aim of the paper is to present the methods developed by the European Chemicals Agency (ECHA) in cooperation with its Committee for Socio-economic Analysis (SEAC) in order to evaluate the impact of costs and effects of different alternative uses of chemicals on the EU population health. The paper will also show the main limitations of the analysis at the EU member countries level.

- Summary/Abstract:The paper presents the impact of manufacturing electric energy on human health. The chosen method of eclectic energy production, on the one hand, is a factor facilitating the improvement of the standard of living; on the other hand, however, it may have a significantly adverse effect on the quality of life. The purpose of the article is to present the impact of energy production from conventional sources and wind turbines on human health and related health costs. ExternE methodology was used to present the health impact of manufacturing energy from conventional sources and to assess the health costs of such a solution. The production of energy from conventional sources poses great danger to human health, thus, one of the alternative ways of providing energy might be switching to wind power – clean, renewable and, most importantly, not causing irreversible damage to the environment and human health. The expert analyses and research used in characterizing the impact of wind power plants on human health unanimously pointed out that, so far, no adverse effect of this method of producing energy on human health has been stated.
