Summary/Abstract: Psychoanalytic paradigm consists of numerous theories about the dynamic nature of psychic phenomena. In this regard organ donation is reviewed in his conscious and unconscious aspects; in its internal conflict dimensions. Clinical experience of working with potential and real organ donors shows a number of peculiarities of the fear, that often play a decisive role in transplantation procedure. Donor situations provoke intense and objectless anxiety, threat of annihilation and disintegration, guilt, and castration fears. At the same time the donor situation is an attempt for reparative integration of the disease of the recipient and the relationship „donor-recipient”. The study presents clinical observations of psychological counseling, psychological assessment and psychoanalytic psychotherapy with living donors of organs for transplantation. Basic fearful experiences of patients on conscious and unconscious level are described.
